‍THIS NOTICE DESCRIBES HOW YOUR MEDICAL INFORMATION MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

At Athlema Physical Therapy, we are committed to protecting the privacy and security of your Protected Health Information (“PHI”). This Notice explains how we may use and disclose your health information, your rights regarding your information, and our legal duties under the Health Insurance Portability and Accountability Act (HIPAA) and Washington State law.

  • Notify you following a breach of unsecured PHI when required by law

We reserve the right to change this Notice at any time. Updated versions will be available upon request and posted in our clinic or website if applicable.

How We May Use and Disclose Your Information

1.      For Treatment

‍We may use and disclose your PHI to provide, coordinate, or manage your physical therapy care. This may include communication with physicians, other healthcare providers, or referral sources involved in your treatment.

‍Although we are a cash-based practice, we may use or disclose PHI for payment-related purposes such as providing invoices, superbills, collecting unpaid balances, or communicating with collection services if necessary.

‍ ‍3.      For Healthcare Operations

‍We may use your PHI for clinic operations including quality improvement, staff training, business management, scheduling, and legal or administrative activities.

4.      Appointment Reminders & Communication

‍We may contact you by phone, text message, email, or voicemail regarding appointments, scheduling changes, or other care-related matters unless you request otherwise.

5.      Uses Required or Permitted by Law

‍ We may disclose your PHI when required by federal or Washington State law, including:

  • Public health reporting

  • Abuse or neglect reporting

  • Health oversight activities

  • Court orders or legal proceedings

  • Law enforcement requests

  • Serious threats to health or safety

6.      Business Associates

‍We may share PHI with third-party service providers who help operate our practice (such as electronic medical record systems or billing services). These providers are required by law to safeguard your information.

‍Any use or disclosure not described in this Notice will require your written authorization. You may revoke authorization at any time in writing.

You have the right to:

  • Request restrictions on certain uses or disclosures

  • Request confidential communications by alternative means

  • Receive an accounting of certain disclosures

  • Receive a paper or electronic copy of this Notice

  • Be notified in the event of a breach involving your unsecured PHI

Requests related to your rights must generally be made in writing.

9.       Cash Pay Restriction Right

If you pay for services out-of-pocket and in full, you may request that we not disclose information about those services to your health plan, and we will comply unless disclosure is required by law.

10.    Security of Your Information

‍ We maintain physical, electronic, and administrative safeguards to protect your PHI from unauthorized access, use, or disclosure.

11.    Complaints

‍If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services. You will not be retaliated against for filing a complaint.
